PHOTOGRAPHY AND VIDEO RELEASE FORM

I, hereby authorize Scott Josephs to use photographs and/or video footage of me taken on [Date of photo/video shoot] at [Location of photo/video shoot], for the purposes of [Purpose of use, such as advertising or marketing].

TERMS AND CONDITIONS

1. Consent: I give my full consent to be photographed and/or videotaped and agree to the use of my likeness in any form of media or advertising.

2. Ownership: The Photographer retains full ownership of the photographs and video footage and has the right to reproduce, publish, and sell them in any way they see fit.

3. Compensation: I understand that I will not receive any compensation or royalties for the use of my photographs and video footage, unless otherwise stated in this agreement.

4. Usage: I understand that my photographs and video footage may be used for promotional or commercial purposes, including but not limited to print and digital media, advertising, and social media.

5. Permission: I give permission for the photographs and video footage to be used without further permission or notice, unless otherwise stated in this agreement.

6. Release: I agree to release the Photographer and their agents from any claims, demands, or causes of action that may arise from the use of the photographs and video footage.

7. Representations: I represent and warrant that I am of legal age, or have the consent of my parent or legal guardian, and that I have the legal capacity to enter into this agreement.

8. Video footage: I understand that if video footage is taken of me, the Photographer may use my voice, image, and likeness, and I hereby grant permission for the use of my voice, image, and likeness in any way the Photographer sees fit.

9. Copyright: I understand that the Photographer owns the copyright to the photographs and video footage and that I have no rights to use them other than as specified in this agreement.

10. Governing Law: This agreement shall be governed by the laws of the state or country in which the photographs and video footage are taken.

11. Severability: If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

12. Entire Agreement: This agreement constitutes the entire agreement between the parties and supersedes all prior negotiations, understandings, and agreements, whether written or oral.

I acknowledge that I have read and understand this agreement and that I have had the opportunity to ask any questions that I may have. By signing below, I agree to be bound by its terms and conditions.
